In-Home Assessment

A SereneCompanion coordinator visits your home. We observe your loved one in their environment and ask detailed questions about their needs, medical history, and preferences.

What Happens

We complete our assessment form (your intake data + our clinical observations). No surprises—we explain everything.

Time

60 minutes

We Design Your Care Plan

Based on the assessment, we create a week-by-week schedule showing exactly what your caregiver will do, when, and how. You approve it before we start.

What Happens

You review the plan. Adjustments are made if needed. You sign off.

24–48 hours after assessment

Caregiver Arrives & Begins Care

We assign a background-checked, trained caregiver to your schedule. First visit includes orientation, intro to your loved one, and setup of reporting/communication channels.

What Happens

Regular care begins with transparent, consistent and open communication. We check in as often as you prefer. Clients and their loved ones are always welcome to reach out with any questions or concerns.
